Why is it so difficult to say to patients that we exactly do not know how biologically depression is caused but we have treatments both medicines and therapy that work good.Please dont keep saying patient that you have low dopamine or serotonin @askdheeraj @chrisaikenmd

Long term rehabs are not effective treatment for addiction. Addiction is best treated in same environment in which all the behaviors have been learned .New coping strategies, rich environment with social connections and family support. @askdheeraj @chrisaikenmd @atulambekar

One major hurdle in psychiatry is google. Patients search medications read about side effects and stop taking it. The basic perception is psychiatrist prescribe "neend ki goli jo kidney kharab karti hain" dont take it. @askdheeraj @sanilrege @atulambekar

In hospitals, we need to have – 1. Casualty 2. Out Patient Departments (OPDs) 3. Casualty where OPD treatment like vitamins is given, as the patient is known to a dumb minister who likes walking around with a camera and telling people how to stand and speak. Or, someone teaches

A patient walked into the casualty at GMC Goa demanding Inj Vit B12. B12 is not an emergency, it can be taken anywhere, even after days of delay. The CMO allegedly directed the patient to a CHC. Turns out, the patient was related to a journalist with direct access to Goa Health

So on " world tobacco day " people just shouting out to stop smoking may not be very helpful. The awareness strategy of how smoking catches hold of a person is discussed nowhere. It is very sad people just mindlessly shouting "STOP SMOKING".

Every act of smoking from the holding it in hand, to taking a puff, exhaling it, the friends and the cup of coffee or tea provides a strong conditioning to the substance. The relation of a smoker to smoking loaded with positive expectations makes it very hard to quit.

Also smoking is not all about nicotine. The harms of nicotine weigh much less than aromatic hydrocarbons and tar. Studies have found even intravenous nicotine does not produce the same effect as smoking a cigarette.

Smoking is one of the most addictive and dangerous substance. The psychological addiction potential of smoking is very high. People try quitting and fail and restart and again quit. This cycle continues, with multiple failures bringing hopelessness. @anikala @atulambekar

Tragic. This isn't the first death in such a 'REHAB'. ADHD kids need proper pharmacological and psychosocial care. Keeping them in rehabs with limited space and movement for just smoking beedis is counterproductive. Behavior problems increase. +
