These observations suggest that weight-bearing isometric muscle force is the key trigger and modulator of POT tremor—not standing per se. The implication: we can discard posture-related balance mechanisms as necessary for tremor generation. This insight opens new possibilities:

To this end, we used a simple yet revealing experimental setup: patients performed isometric leg contractions under an external load using a seated leg press machine. This allowed us to isolate the effect of muscle activation from that of standing.
