Put another way, the reason to do simple cases with the spine endoscope is to become facile with it, which allows you to do more complex cases with greater benefit.

You can’t climb the ladder of competency and tackle complex cases in endoscopic spine surgery without using the spine endoscope to treat much simpler cases. The endoscope is a tool that enables a platform, it is not just a specific procedure.

Can giant calcified thoracic discs be adequately addressed ustilizing an endoscopic approach?. I think they can. These are risky and challenging cases no matter how they are done, but the benefits of an ultra minimally invasive uniportal endoscopic approach for this pathology are

Imagine if lumbar fusion for spondylolytic spondylolisthesis in adults was no longer necessary, or at least extremely rare? . Treating this pathology while there is maximum healing potential instead of using benign neglect may be the next paradigm shift in spine surgery.
