Artificial intelligence may eventually transform healthcare, but, at least for the time being, ChatGPT has neurophobia. #Neurology #neurotwitter #MedTwitter

There you have it, folks. Eastchester clapping sign is your 2023 neurology exam sign champion! Honor its win by asking every right-handed patient with an acute right MCA syndrome to clap. Thanks for all who played along. #neurology #neurotwitter #MedTwitter

In 1879, Sir William Gowers, one of the most prolific clinical neurologists in history, observed that persons with Duchenne muscular dystrophy place 'walk' their hands up their thighs in standing up to overcome weakness of the proximal lower extremity and pelvic muscles.

A High School AP Psychology class at Eastchester High School asked their teacher - "What happens if you ask patients with neglect to clap their hands?" The answer - they perform a one-handed clapping motion to midline. Thus, in 2009, the Eastchester Clapping Sign came to be.

C. Miller Fisher wrote "Some neuro-ophthalmological observations" in 1967 in which he described "one-and-a-half syndrome." Aptly named because there is a conjugate gaze palsy in one direction and a "half" of a gaze palsy in the other due to an ipsilateral PPRF and MLF lesion.

In 1961, Robert Hollenhorst was the first person to write about the direct visualization by fundoscopy of bright plaques in the retinal arterioles. He hypothesized these were due to cholesterol emboli. His observation became critical in patients with cerebrovascular disease.

Just like that, the final four is set! How does your bracket look? Mine is totally busted. Gower over Marcus Gunn? Hollenhorst over Lhermitte? The drama! The madness!

I couldn’t believe that Babinski lost to one and a half syndrome. I bet @ClayJohnstonMD picked that upset correctly in his bracket.

Outside of the historical upset of Horner by Alice in Wonderland (you can't help but think driven by decades of students angry at being questioned about 1st, 2nd, and 3rd order Horner lesions), the remaining top 4 made it in - Hoover, Dix-Hallpike, and Lhermitte.

Gower took down Gerstmann in a show of force from our child neurology colleagues, but otherwise this division has 3 out of the top 4 exam signs competing still: Wernicke, Marcus Gunn, and Jacksonian.

Divison 2 saw a massive upset in Broca going down to Holmes. One of my personal favorites, Eastchester clapping sign (named after the AP psychology class from Eastchester, New York) made it to the sweet 16. Hoffman and Brown-Sequard round out the other signs.

Unsurprisingly, Babinski made it to the sweet 16, blue blood Cushing reflex fell in the first round, and underdog Beevor continues its cinderella story. Romberg fell to Wallenberg, and one and a half round out the signs from division 1 who made it to the sweet 16.
