As an assistant professor Lancet rejected a review from me. I was devastated. 3 months later they asked me to write a review on the same subject. I sent them the same paper without any changes. They accepted it and paid me to publish I! I learnt the review process was nonsense

I am old (73) and if I become delirious in hospital it is 80% likely that is caused by a doctor or a nurse. Health professionals need education and stop thinking physical restraints are the treatment for delirium

Older persons in hospital need to exercise and get out of bed. 1Kg of muscle(roughly a quadriceps) is lost in 3 days of hosptalisation. So many problems in hospital are due to lying in bed #Izquierda #gerotwitter #AGS #geriatrics #aging

I would suggest at least 3 months of geriatrics is necessary for all med students except those going into pediatrics. A similar time should be given to medical residents. There is a nee to recognize that geriatrics is much more than just looking after older adults

SLUMS belongs to US government which was done deliberately by me so it could never be copyrighted or charged for. the same should apply to the Rapid cognitive Screen (RCS) is derived from the SLUMS and should therefore also be free. They both detect MCI as well as dementia

Muscle mass starts to decline at 30 so primary prevention for Sarcopenia starts then. Secondary prevention starts when SARC-F positiv. Tertiary prevention is attempting to reverse disability

My daughter, a nursing home administrator, was in at work at 6 today to give Easter eggs to the night staff as they went home. Those who work in nursing homes work hard and with love for the residents without much appreciation. They are superheros. @wassdoc @macesari

I have known that antipsychotics do not treat delirium for over 30 years because I treated delirium and learnt you do not need drugs. Sad that physicians still use them and with nurses physically restrain patients.will bad health professional behavior ever stop?

There are half as many geriatricians now than in 1990 in USA. That is less than 1 geriatrician per 10,000 older persons. Who will look after me when i am old? We must do a better job at training gps to look after the elderly. consider using the RGA

The role of exercise in medical care is grossly under should be a component of in hospital and outpatient management. We pay for very expensive medicines but we don’t pay for exercise classes. Let’s get our priorities right!

Despite the overwhelming evidence I would argue we still have a long way to go! As far as I am aware #exercise is not integrated into many #medical courses (certainly not in Australia!) and so difficult to be part of medical consult with little or no training!
