6/ The VV can be useful as a rendezvous location when angles around the IJ/SC/BC confluence are thwarting attempts to reach target destination during recanalization. If you find yourself in it, try to get a guidewire into it from EJ or arm – can easily snare and pull through

5/ If uncertain, check steep oblique or lateral view – the VV veers posteriorly towards the spine as you can see in this different patient. The IJ is an anterior structure that is usually relatively in-line with SVC.

4/ From IJ/SC/BC vein confluence, the VV veers posteriorly to follow the vertebral art into the transverse foramen, where it becomes diminutive and more plexus-like. On frontal view can look like a small IJ. Recognize this vein, and never think to put in a snare to stick down on!
