Last thing! Don't forget that invasive meningococcal disease, varicella, pertussis are each nationally notifiable. Lab will generally report cases with positive tests. But cases with clinical diagnosis (w/o lab testing) should also be reported to your local health department.

@jesscataldi Post-exposure prophylaxis (PEP) can get confusing! Should give PEP, but lots of nuances. Generally NOT recommended for the whole class/school. And don't forget about vaccination in the un/under-immunized contacts, and not just azithro.

@jesscataldi Infection prevention • In clinic/hospital: droplet precautions for at least 5 days after initiation of effective treatment (or 21 days after symptom onset if not treated) • School / child care: exclude until 5 days after initiation of treatment

@jesscataldi Treatment: azithromycin x5d is generally recommended. • Early treatment in catarrhal phase may decrease severity and duration of disease • Late treatment may limit contagion, but less impact on symptoms

@jesscataldi Testing: Bordetella pertussis PCR/NAAT from respiratory, nasopharyngeal specimen (it's included in some multiplex respiratory pathogen panels, but not all) • Testing most sensitive in first 3 weeks of cough • Less sensitive after antibiotic treatment

@jesscataldi Remember the stages and complications. Presentation can be different at diff ages. Classic "whoop" doesn't occur in young infants or many older patients, so just in a narrow (but commonly seen) age group. Also, pregnant women can have more severe disease, especially pneumonia!
