#3/3: This can lead others to believe in signs that are not always specific (e.g. hyperacute Ts, SA flag sign). Overcalling STEMI can be dangerous, too. And getting serial ECGs is often the BEST way to go. How about if people start posting more "tough normals"? #foamed.
#2/3: TNs negative, serial ECGs unchanging, patient ruled out for acute cardiopulmonary disease and did well. I posted this bc there is a strong bias towards ECG cases posted on twitter/X that are (often extremely) subtle-positives but that's not real life.

#1/3: Great responses--LAD, SA Flag, hyperacute Ts, swirl, etc. The doc here didn't think about any of those possibilities, and read this as non-diagnostic. He scheduled serial TNs and repeat ECGs. .

Post #3: Here's a comparison of precordial leads in ECG #1 and #2. By itself, ECG #2 is not that concerning IMO; but when compared to #1, it predicts the LAD blockage. #FOAMed
Followup: ECG in prior post was actually ECG #2, non-Dx'ic IMO. But ECG #1 30m earlier here, & now you can see the change. Nicely demonstrates how important serial and comparison ECGs are. Patient coded 20m later, 100% LAD stented, survived. Credit to Dr. Gordon Fuller #FOAMed
