Great case! the IHC is spectacular. i have published 4 cases of syphilis induced renal disease over 35 yrs. this tops them all Am J Kidney Dis. 1987 Feb;9(2):176-9.
Another syphilitic story. Young female w/ untreated syphilis, nephrotic syndrome, severe AKI, and pyuria w/ positive e-coli urine culture. Dx: Secondary membranous nephropathy and pyelonephritis with bonus positive Treponema IHC. Syphilis treated and proteinuria disappeared.

it's not just Hep C anymore. note that lymphoproliferative disorders are about equal to Sjogren as a cause of Cryo, but they may be linked because Sjogren is also related to the development of lymphoma.

😀Thanks @Trumidor!! It seems we have finally convinced nephrologists in our institution to perform kidney biopsies with 16-gauge needle, instead of 18-g. No increase in complications and more accurate diagnosis: obvious benefit for the patient.

This pattern of deposition is most frequently seen on IgG stain in the setting of membranous glomerulopathy which is the diagnosis here.
