This is the MoJ press release from October 27th confirming Lammy's new checks would kick in "from today". Two days later and the Algerian offender was mistakenly released, followed several days later by the second HMP Wandsworth inmate.

David Lammy says Kaddour-Cherif was released before the additional checks he ordered at prisons came into force. But on 27th October he told the Commons the extra checks would be "effective immediately". The MOJ briefed the same. Kaddour-Cherif was released on 29th October.

William Smith was released from HMP Wandsworth due to a clerical error, Sky News understands. He was set free after court mistakenly told Wandsworth his custodial sentence was suspended. By the time a correction got to the right person, he'd already left.
