Challenge away, it’s how we learn! Fair point. Sphincteroplasty shouldn’t be the default first step for any retained stone, but if crushing/basket/etc not sufficient, I’d favor that over continued dilation of a small cystic duct which risks injury or avulsion.

Excellent demonstration! You could technically rescue this - pull apart the two sutures shown by the arrows, the knot will “pop” so you can slide it down and tighten. Used often laparoscopically but would not recommend with monofilament as grasping will weaken the suture.
