Our medicine grab bags have been date checked, reconciled, and restocked ready to support clinicians during patient visits. These bags help stop delays in treatment, especially over the weekend. #VirtualWards #HospitalAtHome #AdmissionAvoidance #UrgentCommunityResponse

Using POCT in community, meant we were able to get a CRP in 4 minutes and put the patient on the right pathway. so were providing IV antibiotics in their own home on our virtual ward! #hospitalathome #virtualward @RotherhamNHS_FT @RotherhamCTD
