Seriously its not hard to find a workable template online, and notaries only charge like $10-15 for a document. Hell some states even have a maximum charge thats pretty low so you don't get ripped off for something simple.

It shouldn’t matter if you work. It shouldn’t matter why you’re unable to work. It shouldn’t matter how “productive” you are. Food should be a basic human right. As a society we should agree not to allow people to starve.
