Our new contribution to the neuropathic facial pain literature, where we are often limited by the treatments we can offer. High cervical intrathecal drug therapy can be considered in refractory patients, even those who have failed other modalities. @painsection @NANS_ION.

The big question: do you need IOM for safety in an asleep patient? Ultimately, probably not, but with the big caveat that this depends on the implanter's skill and comfort. If you think a patient may need it, it can be considered.

Do you need IOM to confirm placement in an asleep patient? No, placement overlying the anatomical midline is enough. Does IOM improve pain? In routine practice, probably not. There's evidence looking at certain associations of dosing and muscle activation, but it's premature.

Proud to see this work online @NeurosurgeryCNS. We looked at patients undergoing percutaneous SCS surgery with or without neuromonitoring. 👉 No difference in stimulation coverage.👉 No revisions for suboptimal positioning.👉 No difference in pain scores.👉 No complications.
Black et al compared placement of #SCS with and without IONM. IONM did not improve outcomes or explant rates, but it did take longer! #Anatomy is the winner!. @NeurosurgeryCNS @StaudtMD .
