SAFE-PVT is the 1st RCT comparing thrombolysis vs urgent surgery for left-sided PHVT. 🟰 Efficacy: similar in both arms.⚠️ Safety: more adverse events in the surgery group. This challenges early surgery as routine strategy. Thrombolysis remains a strong, safe first-line option.

64/M, recent NSTEMI Rx with PCI to culprit Cx 1M back; now brought back for staged PCI to RCA. BG: T2DM with retinopathy/neuropathy, HTN, dyslipidemia, EF 50%. This is the baseline RCA, which we aimed to treat using DEB only strategy (aggressive pre dil with NC & scoring balloon)

2. While this seems a major difference (infact statistically significant), it is important to put this in context with available evidence - where all contemporary stents have shown similar outcomes. Be it Orsiro or Xience or Onyx or Biofreedom, target lesions fail at ~ 6-8% in Y1.

The results show similar efficacy in both arms with worse safety outcome with surgery, thus suggesting to continue with thrombolysis rather than push for redo surgery! Thank you @drkarthik2010 sir for providing me the opportunity to work on this important yet neglected disease.
