It happened again, another hospital that won’t allow doctors to culture a patient with new presenting sepsis who has a central line, catheter or ventilator because if the cultures come back positive they’ll get dinged. But don’t worry they’ll still start 3 antibiotics 🙃

@EJA3232 They literally will hunt the ordering provider down and have a special “meeting” say a pts been in icu a week and now is sepsis positive. No blood cultures or sputum cultures unless you wanna get a write up.

@RNSuperHero I worked at one facility that would culture only if the patient had been in the facility <48 hours. Same with C. Diff testing. Because bottom line over patients every damn time for both hospitals and CMS.

@RN_Atheist Yup! Seen that many times. No cdiff testing on inpatients after 24-48 hours. Treat as if it’s positive 🤦🏼‍♀️

@RNSuperHero That's not CMS guideline treatment of the initial presentation of severe sepsis (this is literally what I do for a living). These institutions will not be reimbursed for their Medicare patients if they don't follow the guidelines...

@RNSuperHero This should be turned in to Medicare & Medicaid for fraud on both the federal & state levels. Ommissive practices to be paid higher $$$ on claims (to avoid penalties) would fit nicely in this category. What is happening is a literally criminal. Can be done anonymously.
