Some places/dynasties also use 'nainai' in place of yitai+taitai but it's not very common in cdramas nowadays. Hope this helps.

For concubines, they're officially not considered part of the clan/family. So they can use their own names in conjunction with yitai/taitai If using yitai/taitai, numbers are left out because they're usually the only woman married to so&so lord/leader with that surname.
