Great MR with @macwalterMD and Previn! A patient with cirrhosis found down…the ddx is endless. Keep salicylate poisoning in mind with a predominant resp alkalosis but a large AGMA, and intubate carefully when a patient is compensating severe metabolic acidosis! #NMPearl

That's a lot of ketones! Great case this morning with Dr. Glowala and Dr. Walter. Bronchiectasis must be differentiated as intrinsic vs traction. Intrinsic has an important ddx you have to work through - CF can fall through the cracks and be diagnosed late!

It’s a beautiful day to start residency. Good luck to our new interns on Day 1! Proud of our new PGY-2s for their hard work all year - no doubt they are ready to be seniors, a big transition. So fortunate for the leadership of our rising PGY-3s. It’s going to be a great year!

Previn led a fantastic MR this morning on MDA5-associated dermatomyositis. Don't be fooled if muscle symptoms are mild or absent. Many thanks to Dr. Santoiemma for the A+ teaching on workup and encouraging us to trust our gut when infection is low on the ddx!

Great case conference with @AnthonyPensa and Dr. Cheema! Fantastic refresher on the ever boards-forward nephrotic syndrome. FSGS and membranous nephropathy are the most common glomerular-first etiologies, but the ddx is long. What a ride!
