Friday with 5 scheduled cases of CRT/CSP. We have experienced all kind of challenges: vein occlusion, difficult CS and branch cannullation, no suitable CS branch required rescue LBBAP CRT, IVCD case with unable to capture the CSP, etc. Each case had its own teaching points.

Be strict when implanting LBBP for CRT. Follow the EHRA Consensus criteria or make sure that resynchronization is achieved using appropriate tools (UHF-ECG, ECGI, septal flash) #LAHRS25 🌐🗺️

I strongly support HBP, but in this case,the great improvement in QRS didn't correct the asynchrony measured with UHF ECG.We opted for LBBAP despite the RV delay.The final programming AV 70 ms and fusion with native rhythm achieved the best result with UHF ECG(very useful tool)

VDI UHF-ECG CE MD installation today at Meilahti Hospital Helsinki. Successful cases, treating both bradycardia patients and patients with heart failure + LBBB by Jarkko Karvonen and colleagues. From the UHF map, you can clearly understand which CRT technique was used 🙂

84 y.o male, complete AV block after TAVR. Permanent non selective His-bundle pacing implanted using pacemapping (His signal not visible most likely due to mechanical damage to the conduction system caused by the TAVR). Perfect synchrony on UHF-ECG. Acute threshold 0.5V @ 0.4 ms.
