Place the probe over the superior lateral aspect of the patella and ask patient to contract their quads. In fat pad impingement cases the fat pad can then be seen to bulge and pop over the Patella under quad contraction. #fatpadimpingement #kneepain #diagnosticultrasound

It’s always worth looking under the CA ligament for joint effusion. Just follow the LHB tendon up past the rotator interval. Keep going ….. you need to angle the probe quite a bit! At the point where the LHB is lost toggle the probe towards the neck and you get to the CAL.

Sonoanatomy is beautiful, especially when you differentiate between different structures using dynamic movements. Moving the patient during an ultrasound scan can give so much more information than just static images #dynamicultrasound

When hunting for an avulsion fracture one hint is the presence of a nice big bruise! Bone is highly vascular and so bleeds when injured, If you see an swollen and bruised lateral ankle sprain go hunting for an avulsion!! #diagnosticultrasound #highanklesprain #anklepain #sprain

An os trigonum can be seen to catch at the posterior ankle joint with dynamic plantar flexion. Dynamic ultrasound assessment can be really useful educational tool to get the patient onboard! #dynamicultrasound #anklepain

I thought I would share this interesting case of both 1st and 2nd dorsal compartment tenosynovitis. The patient has confirmation, via Xray, of radiocarpal joint OA. I have never seen this presentation before. Anyone else seen this before?

Not a common tendon to suffer from a longitudinal split tear however, this patient managed it by playing to much Xbox!!! That's commitment!!!! Confirmed on MRI but beautifully seen with ultrasound, and much cheaper too! #thumbpain #handpain #diagnosticultrasound

The plantaris tendon is incredibly strong and is often spared when the Achilles ruptures however, the question is does management differ when the plantaris tendon remains intact? Thoughts? #AchillesPain #AnklePain #plantaristendon

When do you call joint fluid pathological? This is a difficult question but well worth a discussion. My general rule is, if the fluid causes the capsule to distend of go convex then I feel confident to call it pathological. Thoughts? #diagnosticultrasound #jointpain

The ATFL is essentially a thickening of the talocrural joint capsule and so when it tears talocrural joint fluid can leave the joint space. When the AFTL tears joint fluid can escape and find its way over the talus. #AFTLtear #anklepain #anklesprian #diagnosticultrasound

Rare case of shoulder pain came into clinic a few days ago. An Olympic weightlifter came in with anterior shoulder pain after bench pressing. Ultrasound was unremarkable apart from this insertional pec tendon enthesopathy. #anteriorshoulderpain #pectendon #diagnosticultrasound

Ex-rugby player presented with anterior shoulder pain for two months. Ultrasound showed two significant calcific foci within the long head of bicep sheath. He reports this to be the shoulder that took the brunt of impact during the scrum. #ShoulderPain #LongHeadOfBicep

Dynamic scanning during active or passive dorsiflexion is invaluable. As the ankle moves, the Achilles tendon can be seen gliding relative to the calcaneal cortex and bursal spaces. This motion can help distinguish between tendon and bursal pathology. #MSKUltrasound #Achilles
