CT/FFR has improved diagnostic accuracy when evaluating chest pain patients for coronary artery disease. Outstanding correlation with IVUS/FFR in the cath lab. Will not replace SPECT or stress echo (due to limitations), but it is a fantastic tool.

88 yo with abdominal pain, hematochezia and weight loss. Risk factors of HTN, Chol. Hx of CABG. Abdom. CTA showed densely calcified plaque. C-scope confirmed colitis. Stented the SMA, resolving 60 mmHg gradient. What next?

My observation: iliac vein compression and one additional risk factor account for most proximal DVT’s. Nice result with thrombectomy and stenting.

PE’s often have acute on chronic characteristics in my experience. Large bore thrombectomy (and auto-transfusion) has been a very effective strategy. Photo credit to the tech and the new iPhone.

Tricuspid valve endocarditis complicated by septic pulmonary emboli. Debulked with Angiovac. Treated with Abx and discharged. Months later, embolectomy of bilateral PA’s with FlowTriever.
