Got a call overnight for a patient and as the doc was telling me about the case they said I have never talked to you before but I wanted to let you know how appreciative we are for you and your group cause you guys do such a good job and always help our patients. 🥲.

After a year of being out of training I would say the best thing is to have a stable of people you can ask opinions on to their approach to a case. That even includes people outside your specialty sometimes.

Obstructed pregnant patient nephrostomy .Vertebral body bone biopsy .Celiac plexus block with complete cessation of pain post procedure .Renal cryoablation .Massive pulmonary artery thrombectomy. All on a Tuesday. It’s why I love IR. You can make such a huge impact so many ways.

Due to their age I did not stent. I did use IVUS and ended up plastying. With plan for close follow up and if they develop symptoms again, plan for stenting.

I didn't immediately coil him but I did come back after a few days . MELD came down to 25 and he never developed HE. He ended up getting discharged.

Two recent dilemmas curious what other opinions are. TIPS consult for refractory bleeding MELD of 35. After TIPS do you immediately eliminate the branches? Case 2 patient with MTS do you immediately stent and does patient age influence that decision?
