It's an EPIC day in the life of Jordan Valley CHC! We are making our transition to Epic, a new electronic records program that allows patients to use MyChart - a new way to manage your health information. Thank you for your patience as we work together to use this new system.

MONDAY 7/8: as we improve our electronic health records system, services limited on 7/8. Express Care open 440 E. Tampa & @ Blunt Center, 1720 W. Grand Express Care open 7:30am-4:30pm. Other locations have limited service. Call w/ questions. Regular hours/services resume on 7/9.

Jordan Valley's Chan Ngo Reyes, M.D., has been named a 2024 Health Care Champion physician by SBJ! Dr. Reyes joined the JV team in 2004 and serves as a family physician and medical director for family medicine, express care and vision departments. Congrats!
