“Attached” is right. I literally have never seen a diagnosis removed from a chart. I wouldn’t even know where to start. And that’s how it all gets billed. And then you read a large code-based chart review. And guess what’s being used to train our glorious AI overlords?
Electronic health records are built to optimize billing, not clinical care. A doctor can’t code a note with a diagnosis of traumatic subarachnoid hemorrhage as it’s too generic, according to the computer. But a more generic diagnosis, traumatic brain injury CAN be coded. So

“Less bad” Words every doctor knows by heart
Doctors and scientists should understand that safety is a relative concept. If the risk of disease is 0, then a major adverse event rate of 1 in a million is an unacceptable adverse event rate. For example, there have been no US domestic cases of wild-type polio after 1979, but

By and large, the proper role of doctors is to cure disease, fix injury, and enable people to be healthy. Doctors can't make you healthy. You have to do that yourself.

we need to return to physician led healthcare. We need to allow physicians to own hospitals. We need site neutral payments that pay physicians and hospitals equally. We need expanded HSAs and for patients to pay for the bulk of their care. This includes imaging, labs, drugs,
@realdocspeaks You are doing such great work pointing out the flaws in our healthcare system. Can you post the high points of how to fix?

Term limits for Congress is something people across the political spectrum could agree with. https://t.co/ewyV3u3qrv via @NYTOpinion

Should the USG be printing and borrowing trillions of dollars to subsidize the early retirement of the affluent middle class?
Early retirees like Bill & Shelly will see their health insurance premiums increase nearly 300%—from $442 to $1,700 per month if Congressional Republicans refuse to extend the enhanced tax credits. That's an extra $15K a year families can't afford. https://t.co/Uvo6sYi7YR

.@ReedAbelson @sangerkatz @nytimes thank you. As next year’s Obamacare prices start to become public, Republican lawmakers have returned to a frequent critique of the health insurance marketplaces established under the Affordable Care Act: The law made insurance just too

Amen. Simple, direct, doable.
Everyone that can should buy insurance to insure against catastrophic events. You do not need health insurance for the vast majority of outpatient care. Those that truly need assistance based on income should get a preloaded health card similar to the EBT program.

God bless this man. I don't know about visions, but I do believe that the future is bright - and our optimism will be realized through the enterprise of people like Dutch and countless, tireless friends and mentors at the @councilsns
I love young people. Their enthusiasm and hunger to make a difference are absolutely contagious. Thank you to @JPGK_MD for taking the time to discuss the State of U.S. Healthcare and share his vision for the future of healthcare delivery. The future’s in good hands when people

If we can shop for cars, we can shop for doctors.
@Jimmyspolitics @DrDiGiorgio If the government decided everyone should have a car, would you prefer: A) Government agency selects and distributes cars to the citizenry B) Government agency distributes some minimal "car voucher" that each citizen can use toward the purchase of a car that suits their

Amen. One person's labor is not another person's right.
@Jimmyspolitics It’s still a private good. Without cars or cellphones or food you don’t have a country worth a damn either. All private goods.
