1/2 What is happening here... Two images, acquired 3-4 min apart.....A 61-year-old, HT, AF, normal coronaries; developed chest pain during echo, with MV non-coaptation and florid MR. No change in rhythm or QRS morphology. A few minutes later, all gone.

Let’s look at the timing- AoV flow can never begin before LVOT flow; so, there was no AS, just flail AML with ant-directed severe MR. Pt wrongly advised DVR. Important to always examine valves in gray scale, before jumping on to Doppler.
