To improve care, we need (A) more real world data (trials very difficult in this population), standardised training, and broader, more equitable access to advanced therapies and PERTs.

Clinician confidence varies: 70% deem thrombolysis appropriate for high‑risk PE in pregnancy - but only 20% for intermediate risk. catheter-directed thrombolysis and thrombectomy perceived as useful mainly in high‑risk cases; uncertainty persists in many decisions.

Access to Pulmonary Embolism Response Teams (PERTs) is also inconsistent - 27% have them (often only during limited hours), 47% have none, and nearly 1 in 5 clinicians don’t even know if their hospital has one.

While most centers have access to advanced treatments - 81% for catheter‑directed thrombolysis (CDT), 74% for ECMO, and 67% for surgical thrombectomy - local guidelines & training are patchy: only 1/2 had CDT‑specific protocols, and a mere 28% had training in pregnancy‑related PE

The involving 70 clinicians across the UK & Ireland (Jan–Mar 2024) reveals major gaps in managing pulmonary embolism (PE) during pregnancy. Most respondents were experienced specialists (87% >10 years).
