Endodontic morphological rule:
Nearly all root canal systems are somewhat curved, particularly in a buccolingual direction that we mostly do not properly see.
in this case palatal curve in apical 3rd so, angulated PA helped me to appreciate it.

First of all I'd do sensibility test to confirm pulp necrosis and take angulated PA or CBCT if required to check if it is a lesion or mental foramen
then if it is necrosed I'd take proper history and give it a try Non-Surgically as first line of treatment

got treated as a Direct Pulp Capping by MTA in earlier visit then GIC and followed-up for 2 weeks and more in the coming days inshallah 👍🏻

I can depend on multiple things to assist me to find both teeth canals by taking CBCT first to study case properly then magnification and excellent illumination and depending on krasner and Rankow laws like CEJ and pulp floor color change to find pulp chamber

It depends on multiple factors like
-the cause of exposure is it mechanical or pathological?
-Hx including Pt's symptoms and signs interpreted by a doctor.
-Bleeding or wound nature and its control capability.
-Pulpal tissue intactness and assessment of its vascular supply.

1st visit:pulp extirpation,tooth build-up
2nd visit: cleaning,shaping and MAC PA
3rd visit: obturation of all 3 canals using cold lateral condensation tech and composite filling.

I was lucky to get this beautiful anatomy obturated or filled but it's NOT necessary to do so,otherwise what we should always do is the PROPER Cleaning and Activation of irrigant in each Root Canal System👍🏻

I would do sensitivity test to figure out if it is ONLY perio lesion or endo-perio or perio-endo lesion, CBCT and checking presence of any symptoms about felling of numbness or discomfort due to proximity of lesion to IAN according to findings Tx will chosen
