2/2. Patient is doing well 1 week post-procedure. ❓Some studies show accelerated mucosal healing but thoughts on treating minor defects?.

1/2. ⁉️Novel application of PuraStat⁉️. Case: Minor defect after dual sphincterotomy with 5Fr x 2cm PD stent already in place. Tx: PuraStat applied to create “sealant” & 5Fr x 7cm biliary stent placed to prevent obstruction. Both stents expected to spontaneously pass.

Going through files and found hand written signed original reprints from Basil Hirschowitz, the father of fiber optic endoscopy and ASGE Schindler award winner in 1978. I had the honor of working under his guidance during residency, fellowship, and faculty at UAB Left one 1962.

Part 2 (Pt 1 Nov 4):.🚩Hepaticojejunostomy reached thru prev created duodenojejunostomy.↪️Wire thru perc drain & pulled thru scope.↪️5mm anastomotic stricture dilated w/ 6mm balloon.↪️8mmx4cm FCMS placed.🏁Perc drain removed

Overcoming altered anatomy:.❌Unsuccessful in reaching hepaticojejunoatomy in post-liver Tx to internalize a PTC.↪️Distended afferent limb via PTC.✅EUS-guided duodenojejunostomy with @bostonsci AXIOS to facilitate ERCP . Stay tuned next week for stage 2!

Endoscopic gastroenterostomy stent exchange for chronic GOO due to immune checkpoint inhibitor induced duodenitis (original 2020). ➡️ Endoscopic GE’s are effective options for non-malignant GOO. ➡️Annual exchanges of chronic indwelling stent reduce risk of delaminating.
