Other relative contraindications to thrombolytic therapy include advanced age, internal non-intracranial bleeding in the past few weeks, thrombocytopenia, dementia, elevated blood pressure, and current use of anticoagulant.

...recent ischemic stroke, head injury with bony fracture, and active bleeding diathesis (e.g. hemophilia). Menstruation is not considered an active bleed and thrombolytic therapy during ongoing menses has not generally been associated with significant adverse effects.

Besides hypersensitivity reactions, fibrinolytic agents predispose patients to bleeding. Therefore, absolute contraindications to thrombolysis include a history of intracranial hemorrhage, diagnosed structural cerebrovascular disease (e.g. AVM) or malignant CNS neoplasms, cntd.

Thrombolysis is indicated in cases of hemodynamically unstable pulmonary embolism (massive PE) and is considered in selected cases of high-risk sub-massive PE. These agents convert plasminogen to plasmin, which is fibrinolytic and helps dissolve clots.
