The #UCPexpansion, launching later this year, was co-designed by professionals from clinical areas including end of life care, dementia, frailty, sickle cell, learning disabilities, and paediatrics. We tested the UCP with over 30 professionals from these areas, and urgent care.

Today we launched the EOL candle …as a neutral way of identifying patients who transitioned to comfort care .The candle encourage staff members to modify their words, silence themselves, and reflect….it can be perceived as a symbol of tranquility & respect too ❤️
