💡Use of Nitinol-containing products in patients with nickel hypersensitivity is frequently debated. In case you missed it, @anast_apost et al's DB RCT demonstrates higher rates of chest pain, palpitations, migraines, dyspnoea and rash after PFO closure, but no major AEs.

Higher permanent pacemaker rates when TAVI is performed for AR using JenaValve (~24%) in this multicentre registry. Compare to current experience in AS (~5-10%). Implant depth and oversizing were not predictors. A different paradigm?

First case of co-registration used with OCT at Peninsula Private Hospital. Superb technology. Follow the marker on the angiogram as it travels down the RCA corresponding to the location of the OCT image. Thank you Brian Lynskey from Abbott for case support.

Measurement of plasma omega3 levels acquired from diet is not useful for identifying those at ⬆️ risk of having or developing AVC. Whether high plasma omega3 levels, achievable by high-dose supplementation, may be associated with AVC requires further study.

In this ethnically diverse prospectively-studied cohort who were not routinely supplemented with omega3s, there was no association between plasma omega3 levels (total omega-3, EPA, or DHA) and the presence, severity and progression of AVC.

We hypothesised that higher tertiles of plasma omega3 levels would correlate with a lower prevalence, severity, and progression of AVC, with EPA levels having a stronger association than DHA levels.

We studied the MESA cohort – adults without clinical cardiovascular disease – who had plasma omega3 levels measured at baseline, and who had cardiac CT scans performed serially over a median 9-year period, from which AVC data were obtained.

Mechanistically, ⬆️ omega3 levels could ⬇️ calcification by ⬇️ inflammation, osteoblastic transition of valve interstitial cells, and lipoprotein deposition. In one study, omega3s in aortic valve tissue associated inversely with the degree of calcification.

Aortic valve calcium (AVC) is the precursor of calcific aortic stenosis. Its main mechanistic contributor is chronic #inflammation. #Omega3s are anti-inflammatory. Is there an association between omega3 levels and #AVC? 🧵
