Which may explain why PD patients who have excessive pallidal beta synchronization show hypometric weight shift during gait initiation. Kudos to the entire team and lead author Jessica Bath for spearheading this study!. #Parkinsons #neurophysiology #neuroscience #dbs.

Our results provide evidence that synchronized premotor and motor cortical activities transiently couple with the globus pallidus to regulate the timing of postural responses, and local pallidal activity regulate the amplitude of postural changes during gait initiation.

We found that while pallidal gamma powers are predictive of APA scaling, pallidal-cortical coherence (theta, alpha, beta) and premotor-M1 gamma coherence are predictive of APA timing.

We found 1) beta decrease and broadband gamma increase in the pallidum 2) low frequency power increase in the motor and premotor cortices and 3) increased coherence between the pallidum and cortex during gait initiation compared to quiet standing.

Taking the first step from standing is a fundamental task that we take for granted. However in people with Parkinson’s disease, this task is difficult to perform. Here, we recorded from the motor cortex and basal ganglia of 5 people with PD as they start to walk.

And finally 3) we identified physiological signatures of improved gait with DBS. This work will provide a framework for how we use neurophysiology to optimize neuromodulation to treat motor (and potentially other) symptoms of Parkinson’s Disease.

Here in PD patients implanted with GPi DBS and cortical electrodes, we 1) developed a validated walking performance index to objectively measure gait metrics in PD patients 2) used a Gaussian Process Regressor to identify stimulation parameters that produce the best gait pattern.

While DBS is effective at treating many appendicular symptoms of PD, its effects on gait are variable and less predictable. Why? Because we do not have a good understanding of neurophysiology of human gait and the impact of DBS on gait.
