10/10.Summary.Previously unclear if distal polyps associated with proximal colon cancer incidence. We show for 1st time that distal adenoma size & dysplasia was associated with proximal colon cancer risk. No association found for distal hyperplastic polyp characteristics examined.

9/10.Impact. This is the first study to show an association between distal polyp characteristics & long-term proximal colon cancer risk, helping to inform who might benefit from colonoscopy, and providing evidence for future updates of post-polypectomy surveillance guidelines.

8/10.Limitations. Some subgroup sample sizes were small, limiting statistical power. Further large-scale prospective studies or analyses of pooled flexi-sig trials are needed to confirm these results.

7/10.Findings. No clear association between number of distal adenomas or distal adenoma histological type and proximal colon cancer risk. For those with only HPs, there was no evidence that size or number was linked to future proximal colon cancer risk

6/10.Findings. For distal adenomas, larger size & high-grade dysplasia = higher risk of future proximal colon cancer:.-Adenomas 6-9mm vs <5mm = HR 1.67 (1.07–2.59).-Adenomas ≥10mm vs <5mm = HR 2.08 (0.98–4.43).-High grade vs low-grade dysplasia = HR 2.82 (1.34–5.93)

4/10.Methods. 39,539 participants aged 55-64 underwent flexi-sig screening. Of those, we analysed 9,453 with distal polyps found at flexi-sig, split into:.-4581 w/ adenomas +/- hyperplastic polyps (HP).-4872 w/ HPs only.Proximal colon cancer incidence over 21 years of follow up.

3/10.Background. Flexi-sig reduces distal colorectal cancer (CRC) risk but 40% of new CRCs are found in the proximal colon. We examined whether distal polyps detected during flexi-sig screening were associated with proximal colon cancer risk.
