Personally, I think that CADe is a major advance, and I intend to use it. Finding more adenomas, though a surrogate endpoint, will likely help our patients. But that is one opinion, and the evidence for a standard-of-care recommendation is a high bar — we are not there yet.

What do patients want? We lack data on this. If I told a patient a tool could find more polyps and possibly reduce cancer risk, of course, they’d want it. 👍 If I said it might lead to more colonoscopies without a definite reduced risk of cancer, they might feel differently.

This small and uncertain effect on critical outcomes led the guideline panel to make no recommendation for or against using CADe at this time. This was a spirited debate and was not unanimous — it came down to a vote.

But if it does reduce these outcomes, how many colonoscopies with CADe would you have to do instead of conventional colonoscopy to prevent one additional colorectal cancer? Number Needed to Treat (NNT) = 909. To prevent one colorectal cancer-related death? NNT=5000 🤔
