How to treat POTS?.🔺Fluid and salt .💪 Exercise training.🧦Compression garments.💊 Pharmacotherapies: volume expansion (e.g., fludrocortisone), HR control (e.g., propranolol), vasoconstriction (e.g., midodrine). 🤝Input from others (e.g., Cardiology, Neurology).

Diagnostic evaluation follows same approach for evaluation of DGBI symptoms in general population. Don’t forget!.⭐️Testing for celiac disease.⭐️Testing for functional defecation disorders.⭐️Timely evaluation of gastric motor functions. ‼️Treat the predominant symptoms.

❓Possible MCAS ➡️ episodic symptoms (e.g., pruritic, flushing, sweating, urticaria, wheezing) affecting ≥2 systems. If MCAS suspected, consider:.(1) serum tryptase at baseline *and* 1-4 after symptom flares .(2) Look for 🔺20% above baseline + 2ng/mL.

🩺Test for POTS with postural vitals➡️ symptomatic increase in HR ≥30 beats/min within 10 min. *Consider referral for autonomic testing if refractory GI symptoms + orthostatic intolerance after .❌excluding medication effects and .✅lifestyle/behavior modifications attempted.

✅Use the Beighton score to screen for joint hypermobility

An📈 increase in patients with chronic GI symptoms + hEDS or joint hypermobility + POTS, +/- MCAS. (1) Mechanisms that explain relationships are limited, but evolving.(2) Use targeted testing POTS/MCAS; universal testing not supported by current data.(3) Know when to suspect 🤔.
