No reflow is worse than original occlusion. IVUS = dissection/thrombus/embolism. Normal IVUS; air, microvascular injury or low TM perfusion pressure. IC 💉 must be selective & after MCS “if needed”. 👇 IC 1 mg adenosine, 1 mg verapamil, 0.5 mg nicardipine & integrillin = resolved

90 yrs F STEMI, radial access challenges = small size, stenosis & tortuosity, coronary wire was used to cross while glide wires will not work. Completed diagnostic but needed femoral for pci. One option to use 35 cm slender sheath. Early shift to femoral can be a wise choice.

Best access for cardiac cath in patients with occluded radial artery is the occluded radial artery. Need to access artery DISTAL to occlusion recanalize & insert 23 cm slender glide sheath. #accfit.
