The RATIONS trial was an impressive undertaking. @hannah_rickman and I conclude our letter: 'none of these issues should prevent action to improve food security, strengthen social protection, and reduce the catastrophic costs associated with tuberculosis'. @Tb_JournalClub. [6/6].

I remain a unclear whether households were enumerated before or after randomisation. Whilst the authors claim that the imbalance in cluster sizes was an 'accident', the direction is what might be expected if households had prior knowledge of treatment allocation. [5/n].

Worth noting that this comparison between the clusters (Table S1) uses annualised case finding data from 2022, which is AFTER the trial. Given the intervention appeared effective, we really want to know about how the clusters compared at baseline. [4/n].

Annualised case finding in one of two tuberculosis units called 'Sadar' (an intervention cluster) should have been 2843 not 451 cases. Before the correction, incidence of diagnosed cases appeared 25% lower in intervention clusters. After correction, the arms are balanced. [3/n].

Note, the claim @hannah_rickman and I made about the case notification rates was based on the Supplementary Appendix, which contained an error when first published. The appendix has been corrected since we submitted our letter. [2/n].
