This is a great question. And a big question in any biomedical field is whether we know enough to translate findings to the clinic. Folks will always disagree, but here are my two cents. I think we understand little of human biology and very little of aging, making clinical
@jpsenescence This is the science-vs-technology mindset problem. Scientists are so addicted to learning more and more ad infinitum that they cling to straws of uncertainty rather than trying to exploit what's already almost certain. Medicine is technology; don't pretend it's basic science.

Like aging, there is not much we can do to improve our immune system function - apart the usual healthy lifestyle changes and vaccines for specific diseases. There is no immune system booster, and whoever cracks this will make a big impact. Any biotechs working on this?

There are many gaps in our knowledge, but as others have commented, it's clear that regeneration, plasticity and possibly repair decline with age from early in life. Understanding the mechanisms behind this age-related loss of plasticity and regeneration I think is key.
@jpsenescence So, what do you think are the most promising directions? Feels like there are some large areas of the puzzle missing, and we don’t have the picture on the box. How can we start filling the gaps? Studying immortal species? Germ line?

If aging is caused by damage that only occurs when maintenance processes become less effective post-development then I'd argue that: 1) We don't age due to inevitable, passive accumulation of molecular damage but rather aging is driven by programmatic changes during development
