Antenatal colostrum volumes bear no correlation with postnatal outcomes. Some women choose to hand express colostrum during pregnancy as a 'back-up' for when their baby is born. Any colostrum removed from the breasts during pregnancy will be replaced, it does not 'run out'.

These cause the alveolar epithelial cells within the breast to differentiate into lactocytes, which produce small volumes of colostrum. Colostrum may spontaneously leak during pregnancy - this is of no clinical significance and patients can be reassured this is normal.

Lactogenesis I (or secretory differentiation) begins during pregnancy, and occurs in response to high levels of oestrogen, progesterone, prolactin and placental lactogen.

Lactation is a complex physiological function, dependent on a variety of organs, hormones, pathways and external triggers – much like the rest of our physiology! So, what do doctors need to know?
