Filthy, limp wristing, damaged extractor, damaged extractor spring, extractor cutout area filled with loctite from red dot screw, broken or bent ejector, bad cheap factory ammo, reloads (NO GUARANTEES!) Bad mag, barrel bulge, incorrect recoil spring. Think that's all I've seen
