Demacia is bugged atm, you can get sometimes get 2 radiant items. Chances are if they hotfix it (which they should) it will look a bit different!

Saw metatft had some insight from other regions that have the patch already. Demacia >> everything else? Another b-patch incoming? 🤔 Maybe give it some time but that seems more egregious than multis...
