Excited to represent @DurhamADAPT & #Function_QUERI to share primary outcome results of @chvanhoutven ‘s 8 site trial which is now scaled nationally as #CaregiversFIRST.
Kasey Decosimo from Durham VA. iHI-FIVES CG skills training program decreases number of "not at home" days (e.g. due to ED/hospital/SNF visits). Intervention is now being deployed in all VAs. #BBPgh24
