I agree this needs more nuance. Not all dimorphic fungi are endemic mycoses. We wrote about this topic previously in 2015 and then need to further our understanding.

why don’t we bill for a microbiology read? Cardiology reads every ekg ordered in the hospital, pulm all the pfts and abgs. We also don’t cost share. Oncology often receives 10% of drug costs back. ID has not done this for meds some of which have been equally expensive.
