There are a few specializations in derm you might need a doctorate to get to. Derm path, mohs for instance. But there are very few training positions and even fewer jobs. You could do 2 additional years of post residency training and still mostly practice at the scope of a new PA

But if you just love derm and want the life of a dermatologist, it would be way faster to get a mid level degree and go straight into practice. You can have virtually the same lifestyle and compensation, with probably 6+ years fewer in training.

I understand that if you are already in medical school and it's too late not to do residency, and you want a great lifestyle specialty and relatively short training, derm is great. 4 years is shorter than the 6 it would take to get to cards, gi, or rheum.
