What if patients don't care about consciousness or empathy? What if they just want the job done correctly? . We're already seeing AI companions fill emotional needs elsewhere.

The question isn't whether this future will arrive. It's whether we'll shape it thoughtfully, preserving human agency where it matters most while optimizing for the greater good. The timeline may be shorter than we think.

This isn't about fearing disruption. It's about glimpsing what lies beyond: dental equity at unprecedented scale. If a child in Sudan could access the same quality care as a patient in Europe, wouldn't that justify the transformation?.

The global implications are staggering. Over 3.5 billion people lack access to basic oral healthcare (WHO Global Health Observatory, 2023). Imagine AI systems reaching underserved populations where no dental care existed before.

This raises Kurzweil's question about machine consciousness and emotional intelligence. But here's the uncomfortable reality: What if patients don't care about empathy? What if they just want the job done correctly?

The evidence is already here. Recent studies show AI can plan implant cases with perfect consistency while human outcomes vary by education, experience, even mood (Elgabra 2025). No fatigue, no bias, no off days.

Using Peter Thiel's "Zero to One" framework, AI might be the first horizontal innovation that actually replaces the core human role rather than enhancing it. We're moving from tools to agents (Thiel, "Zero to One").

Throughout dental history, horizontal innovations made us more capable. Anesthesia, osseointegration, digital workflows all preserved us as fundamentally dentists doing dentistry, just better. But this time feels unprecedented.

AI just achieved 100% consistency in implant planning while humans have "bad days." For the first time in dental history, we're not just getting better tools but facing potential replacement. Here's why this breakthrough feels different.

Previous dental innovations have made us more capable. Anesthesia, osseointegration, and digital workflows have enabled us to do better dentistry as dentists. AI represents something unprecedented: agents, not tools.

When you force a deep learning model to compress CBCT scan data into segmentation masks, you lose the subtle patterns that matter most. End-to-end training preserves what compression destroys.

Although 99% of the cases I perform are with Guided Surgery, I recommend starting with "piloted surgery" and slowly start moving into guided surgery. And that´s it for today.

I think everyone should use at least “piloted Surgery,” which means having the case prepared with a digital mock-up, a guide for the pilot drill. This will push everyone to have a well-prepared and diagnosed case.

If any “beginner” chooses Guided Surgery, they should learn how to perform guided bone regeneration or the basics of soft tissue management, which would make them “no beginners” anymore.

Rule-based algorithms in medical imaging are basically expert-defined decision trees. But if human experts could articulate clear rules for complications, we wouldn't need AI in the first place.
