tl;dr: - Although BV score differentiates well between bacterial and viral RTI —> use with caution in “atypical” pathogens like Mp - Although imperfect in Mp —> microbiological testing remains important - These findings come in time as Mp resurges —>

Finding #2: Combining clinical features (age, duration of fever, duration of resp. symptoms) with BV performed as well as combining them with PCT (AUC 0.86 vs. 0.87) in terms of discriminatory accuracy 6/n

Findings: - BV score was significantly lower in Mp+ CAP than in MP- CAP (median 14 vs. 54), looking more like a “viral” pneumonia - In line with this, TRAIL levels were higher and CRP lower in Mp+CAP than in Mp-CAP 5/n

First, some facts about Mp: - “Atypical” bacterium (e.g. lacks cell wall) - “Atypical” presentation - Hard to diagnose (serology is unreliable, PCR does not discriminate infection from carriage) - Almost disappeared during the Covid pandemic, but seems to be resurrecting! 2/n
