There's an urgent need for integration of TB response into national, regional & global climate response strategies, esp in areas w high burden of TB. Failure to act will not only stall progress to #endTB but will also exacerbate global health inequities and social disparities.

We reviewed the literature for key pathways, including food and water insecurity, displacement, and disrupted healthcare access. We found extensive evidence supporting the existence of plausible links between climate change and TB.

We drew on existing knowledge of TB determinants, identified those likely to be sensitive to the effects of climate change, and conceptualised mechanistic pathways through which this might occur.

In our letter @GlobalHealthBMJ, with incidence of 170,000 we found displaced people bear ~2% of global TB incidence burden, & 4% of TB deaths. TB also accounts for 3.5% of all deaths in IDPs. As rates of displacement continue to climb this could reach 11% of all TB deaths by 2035.

In 2024, 68million people were displaced by humanitarian crises, & are at increased risk of TB, the world's biggest infectious disease killer. We urgently need estimates to identify the scale of this problem & drive our response. We provide initial numbers

At end of treatment: we recommend reassessing anthropometric measures to assess nutritional recovery. Those who remain underweight should receive close follow-up to detect early relapse.

During treatment: we recommend weight monitoring, where lack of weight gain at end of intensive phase should prompt investigation of food insecurity/poor treatment adherence/malabsorption/uncontrolled diabetes/drug resistance.

At treatment initiation: we recommend standard nutritional assessment with anthropometric measurements & haemoglobin estimation, as well as macronutrient & micronutrient support with counselling.
