We contextualize key studies (complete + ongoing) and provide plausible explanations for contemporary utilization in the absence of randomized (AC-controlled) evidence.

Congratulations to @MedChiefs resident Dr. Ryan Sachar, and @kychangmd, on their presentation at #AHA24! Patients screening positive for HFpEF with @ultromics AI algorithm had higher PA pressures, c/w more advanced disease

Dear enemies,The Black Swan is now the most influential (that is, discussed/mentioned) 21st C book. While almost all books have a half-life between 2 and 6 months, it does not appear to have a half-life. #Lindy [Same with other 4 books in the Incerto. Graph is US only]

I think the field of medicine might improve if we 100% stopped considering research when selecting candidates for med school and residency. Those who love research would keep doing it (good). Those who don't love it would stop doing junk research (also good). Research might go

It's really sad that so many young people who want to go into medicine get tricked into working for pipetting laboratories. Then we make them lie and say they enjoyed the pointless hazing that has nothing to do with clinical medicine.

PBMs say they don't set drug prices, but their own data disagrees. For example, Medicare data for CVS Health plans show them setting 597 different prices for imatinib that are as low as $2,658 and as high as $8,199. @mcuban sells the same drug for $40. https://t.co/iPL1VkTmzL

I have had at least 5 patients (in residency) who are otherwise either (a) fine or (b) have bigger problems, come to clinic saying they have PAD because apparently managed Medicare knows more about screening than we do

My mother called me in a panic (and how that call helped me understand Medicare Advantage): "Robert, they told me I have bad blood flow in my legs. What do I do?!" Woah. Wait, what? Who told you that?

Patients who were admitted for a week with sepsis requiring abscess drainage, a fib, and a kidney injury don’t need 12 pages of “heart healthy eating tips” in their discharge paperwork. They need to be able to see their instructions, meds, and appointments simply and clearly.
